How can I design an ideal subscription form?

Here are some tips goes in the making of an ideal subscription form:
•The form should be as simple as can be.
•It should be short in length so that users can fill in easily.
•Users should know what they are signing up for. The form should       
take care of this.
•There should be no more than 7 points of fill in blanks
•Use drop down boxes to fasten things.

Also try and use CAPTCHA with the form as this will help keeping the 
spammers away and make the use of the form more effecient.
